How to Disenroll

You can end your membership in Meridian Medicare-Medicaid Plan (MMP) at any time. The change will become effective the first day of the next month after we receive your request. If you do decide to end your membership with Meridian, you should call Client Enrollment Services for the Illinois Department of Healthcare and Family Services at 1-877-912-8880 (TTY 1-866-565-8576), Monday - Friday from 8 a.m. to 6 p.m. The call is free. 

If you request disenrollment, you must continue to get all medical care from Meridian until the effective date of disenrollment. Contact us at the phone number below to verify your disenrollment before you seek medical services outside of the Meridian network. If you enroll in another Medicare-Medicaid Plan, Medicare Advantage Plan or Medicare Prescription Drug Plan, your membership in Meridian will generally end unless you are receiving certain services from Meridian.

Your call will be returned within the next business day.
